I, ; , as the data subject (hereinafter referred to as the Data Subject) within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter GDPR), explicitly confirm that as the Data Subject I am:

• aware of and agree that the DIOCESE OF GOSPIĆ-SENJ, Senjskih žrtava 36, 53 000 Gospić, email: info@manreza.hr, phone: 00385993810077 (hereinafter the Diocese), processes my personal data for the purpose of my participation in (hereinafter the Spiritual Exercises), especially for the purpose of better recognizing my spiritual affinities and enabling more effective participation in the Spiritual Exercises,
• aware of the term personal data and understand that it means any information relating to me as an individual through which my identity is established or can be established, such as, but not limited to: identification data, contact information, and other data necessary for participation in the Spiritual Exercises,
• aware of the term processing of personal data and understand that processing means any operation or set of operations performed on personal data or on sets of personal data, whether or not by automated means, such as collection, recording, organization, structuring, storage, adaptation or alteration, retrieval, consultation, use, disclosure by transmission, dissemination or otherwise making available, alignment or combination, restriction, erasure, or destruction,
• aware that data revealing racial or ethnic origin, political opinions, religious or philosophical beliefs, or trade union membership, as well as genetic data, biometric data for the purpose of uniquely identifying an individual, health data, or data concerning a natural person’s sex life or sexual orientation, represent a special category of data under Article 9 of the GDPR,
• agree and hereby give my consent for the Diocese to process my personal data, including my personal data considered special category data under the GDPR, in particular but not limited to my religious beliefs and health-related data,
• aware of all my rights under the GDPR, in particular:
  • the right to be informed about the processing of my personal data under Articles 13 and 14 of the Regulation;
  • the right of access to my personal data under Article 15 of the Regulation;
  • the right to rectification of my personal data under Article 16 of the Regulation;
  • the right to erasure ("right to be forgotten") under Article 17 of the Regulation;
  • the right to restriction of processing under Article 18 of the Regulation;
  • the right to data portability under Article 20 of the Regulation;
  • the right to object to the processing of my personal data under Article 21 of the Regulation;
  • the right not to be subject to a decision based solely on automated processing under Article 22 of the Regulation;
  • the right to lodge a complaint with a supervisory authority under Article 77 of the Regulation;
  • the right to an effective legal remedy against the Diocese under Article 79 of the Regulation.
• aware that I may at any time withdraw this Consent by contacting the Diocese either by email or by post, with a unilateral written statement of intent, and that I am not obliged to provide this Consent,
• aware and agree that my personal data may be transferred to third parties with whom the Diocese cooperates for the organization, implementation, and participation in the Spiritual Exercises, including but not limited to Internet providers, business partners providing administrative support, employees and officials of the Diocese, and other persons regularly in contact with the Diocese regarding the Spiritual Exercises, as well as other external collaborators.

By signing this Consent, I confirm all of the above and give my clear and explicit consent to the processing of my personal data as stated herein for the purpose of participation in the Spiritual Exercises, in accordance with Article 6(1)(a) and Article 9(2)(a) of the GDPR.

• agree and hereby give my consent for the Diocese to process my personal data, including special category data under the GDPR, particularly but not limited to my religious beliefs and health-related data,
• aware of the role of a Companion in the Spiritual Exercises, meaning a person who communicates weekly with the Data Subject, who has access to certain special categories of personal data about the Data Subject, who assists the Data Subject in the process of the Spiritual Exercises, who guides the Data Subject through them, and who informs the Data Subject about events within the Spiritual Exercises (hereinafter the Companion),
• agree and hereby give my explicit consent that the Diocese transfers my personal data, including special categories of data, to the person assigned to me as Companion in the Spiritual Exercises,
• aware that I may withdraw this Consent at any time by contacting the Diocese either by email or by post, with a unilateral written statement of intent, and that I am not obliged to provide this Consent.
